The Clinical Pharmacist provides individualized, cost-effective pharmaceutical care and pharmacy operations within a healthcare team to optimize patient medication outcomes. This role requires a PharmD or Bachelor's in Pharmacy and relevant state licensing, preferably with acute care hospital experience. The Clinical Pharmacist has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.
Performs all duties in a manner, which promotes team concept and reflects the Sisters of Providence mission and philosophy.
